Job Description:
A company is looking for a Remote LPN Personal Care
Coordinator.
Key Responsibilities
Conduct monthly care management conversations via outbound phone
calls to address patients' healthcare needs
Communicate available resources and services to patients throughout
their care continuum
Maintain chart compliance and documentation, including medical
history and care plan adjustments
Required Qualifications
Current active, compact LPN/LVN license
LPN/LVN degree from an accredited nursing program
Ability to follow established clinical protocols and procedures
Flexibility in task management and switching tasks effectively
Willingness to assist co-workers and contribute to team success
