Job Description JOB SUMMARYThe Licensed Vocational Nurse
provides case management for an assigned group of clients including
coordinating and monitoring the activities of the treatment plan.
The incumbent will complete and oversee assignments to improve the
diet, nutrition, and wellness of the
clients.RESPONSIBILITIESReasonable accommodations may be made to

functions of the job.• Provides service coordination for an
assigned group of clients; Assumes responsibility for developing,
writing, implementing and evaluating overall treatment goals in
conjunction with the treatment team.• Provides individual
supportive counseling and symptom management; Educates and support
clients and family members.• Conducts comprehensive assessment of
psychiatric history and physical health needs as part of the
treatment team.• In collaboration with the treatment team,
participates in the organization and management of medication
coaching and delivery to clients, as well as integrating medication
administration into treatment plans.• Assists RN and MD in
medication oversight and accounts for all prescribed medication;
Responds appropriately to medical emergencies. Ensured physician
orders are carried out and coordinates medical care.• Develops
curriculum and prepares manuals, visual aids, course outlines and
other materials used in promoting health, and provides individual
and group wellness coaching and education.• Coordinates with
primary physicians and health care personnel to determine physical
capabilities and nutritional needs. Conducts health and wellness
screenings/assessments for assigned clients.• Assists in
administering Patient Assistance Program, ensuring efficiency and
accuracy in all aspects.• Provides on-call crisis intervention
covering nighttime, and weekend hours as needed.• Completes all
charting and documentation as contractually required as dictated by
program requirements.• Ensures that all medical policy and
procedures are followed and meets regularly with Program Leadership
to monitor this. Assists Psychiatrist in administering and
evaluating medications prescribed by Psychiatrist, evaluating
medication effectiveness, side effects, and complications. Arranges
for lab work according to protocol.• Participates in regularly
scheduled staff meetings, Daily Organizational Staff Meetings and
scheduled URC meetings.• Comply with all MHS and program rules,
policies and procedures including HIPAA standards..• Prepares
critical care issues for discussion during supervision and Daily
Organizational Meetings.• Meets productivity requirements and
documentation standards, as established by the County, MHS and the
program.• Provides referrals and linkages to community medical
treatment, and acts as liaison and advocate on behalf of the client
with medical providers
QUALIFICATIONS• Completion of a licensed Vocational Nursing Program
from an accredited college or university• Current licensure as a
Vocational Nurse in California• Minimum of three years of
experience treating psychiatric and dually diagnosed population•
Bilingual English/Spanish if the position serves Spanish speaking
population highly desirable• Demonstrated ability to provide
counseling and clinical services. Ability to communicate verbally
and interpret, explain and advise about medical information to
clients, staff, family members and community providers• Ability to
work independently and effectively collaborate as part of a team•
Ability to problem solve and react appropriately to emergency
situations• Ability to create written documentation that is
legible, clear and concise• Demonstrated knowledge of cultural and
socio-economic factors that inform service provision to persons
with psychiatric or dual diagnosis disorders; demonstrated ability
to engage culturally diverse populations• Proficient with Microsoft
