Job Description:
Assisted is looking for an experienced Hospice Liaison to
educate patients on hospice services and help grow our Mission
Hills branch! This position is eligible for a $3,000 sign-on bonus.

this position at commencement of employment is expected to be
between $80,000 - $95,000 annually; however, base pay offered may
vary depending on multiple individualized factors, including
job-related knowledge, licensure, skills, and experience. There is
also an uncapped commission structure for this position in addition

you are: At least 1 year of outside sales experience Hospice, home
health, home care, or DME experience is preferred. Self-driven
independent multi-tasker with outstanding customer service skills
What you will do: You will have a unique opportunity to help to
grow our business and impact the bottom line! You will be
responsible for identifying appropriate accounts/patients through
review and consultation with health care professionals to assist in
securing appropriate services and follow-up support. You will help
to achieve and maintain continuity of patient care by planning and
exchanging information with other professionals within the
hospital, long term care facilities, physician offices, medical
groups, case management offices and others in the community. A full
