Job Description:

Family Health Center of San Diego is looking for a Physical Therapist to provide evaluation and treatment for PACE participants to determine mobility level and other physical condition and functional abilities. Provide and recommend therapeutic exercise, gait training, ambulation, etc. Plan and develop individual and group exercise programs for maintenance therapy for participants. Responsible for providing a comprehensive physical therapy program to include development and oversight of functional maintenance programs.

Responsibilities:

- Identify participant's overt problems/needs and set priorities.

- Develop a plan of care based on physical therapy process which incorporate the plans of other disciplines and continuing care needs to include the participant/family in developing or revising the plan of care. Make referrals to interdisciplinary support services.

- Establish realistic and measurable goals and set up treatment plans to administer rehabilitation treatment. Communicate participant changes to interdisciplinary team.

- Collect participant data and complete required forms to ensure compliance with regulatory requirements.

- Understand and demonstrate respect for participant's rights and utilize established mechanisms for management of ethical issues.

- Make home visits for necessary home-based interventions and observations.

- Integrate cost effective measures into rehabilitation practice. Active involvement and participation in QI processes in complying with policies addressing safe working conditions.

- As an active and cooperative member of the interdisciplinary team, attend meetings and actively participate in the care planning of the PACE participant.

- Perform other duties as assigned.

Requirements:

- Bachelor's degree in Physical Therapy required; Master's degree in Physical Therapy preferred.

- Current California state licensure as a Physical Therapist.

- Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.

- 1-2 years of experience working with the older adult population.

- Clinical competency with the older adult population preferred.

- Traveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver's license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver's license within ten days of establishing residency. Mileage and other reimbursement governed by policy.

70,640.00 - 101,537.75

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
